- Your Name
- Your Player’s names and email addresses (you may supply this later if you like)
- Your preferred payment method (Check or CC via Paypal)
- Matt will reply to you and confirm your registration along with the payment instructions. Do not wait to register if you know you will have a team. There are 30 team spots available and we sell out every year.
Once again we return to The Oaks with a 1pm shotgun Saturday afternoon. The site of 6 of our previous events, The Oaks features a dramatic layout that will challenge golfers while allowing for great opportunities to socialize with other teams. The scramble format allows golfers of all skills to enjoy themselves while contributing to a great cause. Your entry includes a buffet lunch, golf range, a goodie bag and a donation to LLS and of course – 18 holes of fun filled golf!

Hole Sponsors: Can’t make it? That’s ok – for $300 Your Company Name and Logo will prominently appear on a 2’x3′ corrugated sign.
Beverage Sponsor: $500 Your Company Name and Logo will appear on one of two beverage carts
